Community Healthcare of Texas is currently recruiting for a Field LVN.

$2,500 Sign-on bonus

POSITION SUMMARY

Assist the RN Case Manager in the plan of care for the patient.

ESSENTIAL FUNCTIONS

  • Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice.
  • Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given.  Record pain/symptom management changes/outcomes as appropriate.
  • Administer medications and treatments as prescribed by the physician.
  • Document nursing care and significant interactions with patients, support persons, and team members.
  • Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact.
  • Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
  • Participate in on-call rotation

POSITION QUALIFICATIONS

  • Licensed Vocational Nurse licensed by the State of Texas, required 
  • Hold a valid driver’s license and have a dependable means of transportation
  • 1 year of nursing, hospice, or oncology experience preferred
  • 1 year LVN experience in an acute care setting required.
  • Proficient skills using a computer are required; Microsoft Office suite experience preferred
