PRN Physical Therapist – Home Health
Join a team that values flexibility, autonomy, and patient-centered care. We are seeking a compassionate PRN Physical Therapist to provide exceptional in-home therapy services that help patients regain mobility, reduce pain, and improve overall function and quality of life.
As a Physical Therapist, you will:
• Perform and document comprehensive evaluations to identify patient needs and develop individualized treatment plans
• Plan and provide therapeutic interventions to restore function, improve mobility, relieve pain, and prevent disability
• Monitor patient progress and modify treatment plans to maximize outcomes
• Maintain timely, accurate documentation per company guidelines
• Communicate effectively with patients, families, physicians, and interdisciplinary team members
• Follow infection control and universal precaution standards
• Support agency philosophy and commitment to quality patient care
Requirements:
• Graduate of an accredited Physical Therapy program
• Current Physical Therapy license
• Minimum 1 year home health experience preferred
• CPR certification
• Valid driver’s license and auto insurance
• HomeCare HomeBase experience preferred
Physical Demands:
Must be able to stand, walk, bend, squat, kneel, and lift/push/pull up to 100 lbs as needed.
