We are seeking a motivated and compassionate Case Manager to support veterans in obtaining and maintaining meaningful employment. This role focuses on building strong relationships with both veterans and local employers to create job opportunities that align with each client’s skills and goals. A key component of this position is providing ongoing support after placement, including coaching, problem-solving, and regular follow-up to ensure long-term job retention and success. The Case Manager will play a critical role in helping veterans overcome barriers to employment and achieve greater independence and stability.

 

Key Responsibilities

  • Assess employment readiness, skills, and career goals
  • Develop and implement individualized employment and retention plans
  • Build relationships with local employers to create job opportunities
  • Provide job readiness support (resume, interview prep, job search)
  • Offer ongoing coaching and follow-up to support job retention
  • Coordinate with employers and community partners
  • Maintain accurate documentation and track client progress
  • Advocate to reduce barriers to employment
Qualifications
  • High school diploma with social service or human services experience. Bachelors degree preferred.
