Heritage Crossings is looking for a dedicated Physical Therapist Assistant (PTA) to provide high-quality rehabilitation services on a PRN basis. Working alongside an experienced therapy team, you'll help residents improve mobility, restore function, and achieve meaningful outcomes in a supportive senior living environment.

How You'll Make an Impact

  • Implement individualized treatment programs developed by the supervising Physical Therapist.
  • Support residents in reaching their rehabilitation goals through therapeutic exercises and functional mobility training.
  • Monitor resident performance and communicate progress, concerns, and recommendations to the therapy team.
  • Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
  • Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.
  • Encourage resident participation while fostering confidence, safety, and independence.

What We're Looking For

  • Graduate of an accredited Physical Therapist Assistant program.
  • Active Nebraska Physical Therapist Assistant license.
  • Strong clinical, communication, and organizational skills.
  • Ability to build positive relationships with residents, families, and team members.
  • Desire to provide person-centered care with professionalism and compassion.
