This position is accountable to the Program Director of the inpatient rehabilitation program. The main job duties are: 1.) to respond to, and carryÂ
out, pre-admission screenings for patients referred to the rehabilitation program, 2.). to assist in entering referral and admission data into aÂ
database, 3) compile monthly and quarterly trend reports and analyzes the data with recommendations for improvement or continued success andÂ
4) to provide education regarding inpatient rehabilitation to a variety of stakeholders, 5) has the overall responsibility of evaluating and consultingÂ
with PM&R physician in determining if patients meet criteria for admission to Acute Inpatient Rehab
Responsibilities
- Responsible for the assessment of patients referred for inpatient rehabilitation services; makesÂ
recommendations for alternate levels of care within the continuum to referral sources if the patient is not aÂ
candidate for admission . These assessments wll be internal or external and may require travel to hospitals,Â
physician offices, patient residence or emergency rooms. Will enter all required referral and admission data intoÂ
the provided computer evaluation tool and database - Coordinates the gathering of pertinent patient information on referred patients from various sources, includingÂ
the medical record, onsite and telephonic interviews, and through other electronic media . Consults with theÂ
Admissions Coordinator to enable referral to advance through admissions process. - Communicates quickly and effectively with all referral sources at every stage of the referral process as a meansÂ
to achieve referral source satisfaction and to advocate for patients by providing a prompt decision regardingÂ
admission . - Initiates the flow of patient information to the Program Director and Medical Director for review and decisionmaking.
- Initiates the admission of patients by fully completing all necessary forms and insurance pre-certifications.Â
Seeks out pre-certifications when appropriate to patient needs. - Other related job tasks or responsibilities as assigned
Qualifications
- Special Skills : Â Ability to comprehensively review a medical record for inpatient rehabilitation medical necessity; routinely demonstrates goodÂ
organizational skills; is results-oriented and independently goal-directed; is able to multi-task and able to communicate well with many diverseÂ
audiences - professional and non-professional; is able to meet established timeframes; demonstrates good customer service skills; is able to workÂ
independently; is able to pass the FIM competency and pre-admissions screening competency examinations; has experience with computers andÂ
other electronic data entry devices; must have valid driver’s license. Travel with own vehicle required, eligible for mileage reimbursement withÂ
appropriate tracking submitted. Must be able and willing to travel via own personal vehicle for in town in the Phoenix Metropolitan Area at least 50%Â
of time. Travel reimbursement provided- Minimum RequirementsÂ
- Experience:Â Must have a minimum of 2 years of clinical experience, with clinical expertise and strong interpersonal skills having beenÂ
demonstrated; experience in rehabilitation setting preferred. - Education:Â Graduate from an accredited nursing program as an RN or has other clinical licensure as a PT, OT or SLP
- Certifications: Â Registered Nurse Licensure or Clinical licensure required applicable to the provision of inpatientÂ
rehabilitation services; PT, OT, SLP
- Experience:Â Must have a minimum of 2 years of clinical experience, with clinical expertise and strong interpersonal skills having beenÂ

- Experience:Â Previous demonstrated experience and success working in a self-directed capacity. Prior Supervisor experienceÂ
desired. - Education:Â Bachelor's Degree
- Certifications:Â For RN's: CRRN or eligible for CRRN certification exam (requires a minimum of 2 yearsÂ
experience in an acute inpatient rehab facility. FIM credentialed or certifiedÂ
- Experience:Â Previous demonstrated experience and success working in a self-directed capacity. Prior Supervisor experienceÂ
