*This is a full-time position offering a Monday-Friday schedule and requires travel across the West/Central area
As a Licensed Practical Nurse (LPN), you鈥檒l provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Key Responsibilities
路聽聽聽聽聽聽聽聽 Deliver compassionate, direct patient care in accordance with state regulations
路聽聽聽聽聽聽聽聽 Implement care plans developed by the RN and document progress accurately
路聽聽聽聽聽聽聽聽 Monitor patient conditions and communicate observations to the RN and physician
路聽聽聽聽聽聽聽聽 Collaborate with the care team to ensure seamless, high-quality service
路聽聽聽聽聽聽聽聽 Participate in professional development and team meetings

Requirements
路聽聽聽聽聽聽聽聽 Graduate of an accredited practical/vocational nursing program
路聽聽聽聽聽聽聽聽 Current LPN/LVN license in the State of Arizona
路聽聽聽聽聽聽聽聽 3 years of nursing experience preferred (Home Health or MedSurg is a plus)
路聽聽聽聽聽聽聽聽 Current CPR certification
路聽聽聽聽聽聽聽聽 Valid driver鈥檚 license, auto insurance, and reliable transportation
