*Provide comprehensive assessments, evaluations, and diagnostic summaries of chemically dependent patients.
*Compile Patient centered, comprehensive treatment plans, meeting the regulatory time frames.
*Document Pt. progress towards the goals of treatment within OASAS regulatory standards.
*Conduct education and therapeutic treatment protocols focused on Pt. need and OASAS regulations.
*Provide case management services, in behalf of Pt. with all collateral agencies. Provide necessary reports to TASC, Drug Court, Parole, DCMH, DSS, CPS.
*Complete discharge summaries in the OASAS time frames, with appropriate aftercare in place.
* Bi-lingual in Spanish required
PAY: $35-42hr
