A great healthcare facility located near Rexburg, Idaho, is seeking an experienced Registered Nurse to join their team as the Lead Registered Nurse Case Manager

Lead Registered Nurse Case Manager Opportunity:

• Full-Time, Permanent, Direct-Hire
  
• Schedule: Monday - Friday
  
• Setting: Home Health

• The Lead RN Case Manager will develop and update individualized patient care plans. Train new team members, model compliance with medication conditions of participation, and act as the primary point of contact for the care team.
  

Lead Registered Nurse Case Manager Qualifications:

• Must have or be eligible to obtain an Idaho RN license or Compact RN license upon employment
  
• Must have or be eligible to obtain BLS Certification
  
• Must have at least 1 year of acute care and home health nursing experience
