Responsibilities

Coordinate patient care plans to ensure quality and continuity of care.
Assess patient needs and develop individualized care plans.
Monitor patient progress and coordinate appropriate healthcare services.
Collaborate with physicians, nurses, and interdisciplinary healthcare teams.
Assist with discharge planning and transitions of care.
Advocate for patients and help ensure access to appropriate healthcare resources.
Maintain accurate patient records and documentation.
Ensure compliance with healthcare policies and procedures.

Qualifications

Active Registered Nurse (RN) license.
Minimum of 1 year of nursing experience.
Previous case management experience preferred.
Strong communication, organization, and care coordination skills.
Ability to work independently and as part of a healthcare team.
Must successfully pass a government background check.
Ability to meet all facility credentialing requirements.
