St. Mary's Medical Center is looking for a casual Discharge Planning Specialist to assist with discharge planning responsibilities.
The role involves coordinating discharge services such as home health and rehab placement under the supervision of a Care Facilitator or Social Worker.
Candidates are required to have a High School Diploma, with preference given to those who have attended Junior College or Technical School.
Experience in hospital healthcare transitional care and discharge planning is preferred, along with a BLS certification and a working knowledge of medical terminology.
The position requires excellent verbal and written communication skills to effectively interact with patients, families, and hospital staff.

St. Mary's Medical Center is seeking a casual Discharge Planning Specialist. The Discharge Planning Specialist (DPS) will assist the Care Facilitator and or Social Worker with the discharge planning responsibilities, to promote timely and safe discharge services. Under the supervision of the Care Facilitator and or Social Worker, the Discharge Planning Specialist facilitates and coordinates discharge services such as home health, DME, Rehab placement, and other community resources or agencies.

Qualifications

Education Requirements:

High School Diploma is required. Junior College/Technical School preferred.

Experience:

Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.

Certifications/Skills:

BLS required. Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.
