About the Role:

The Case Manager plays a pivotal role in coordinating and delivering comprehensive support services to individuals and families in need, ensuring their well-being and stability. This position involves assessing client needs, developing personalized care plans, and facilitating access to appropriate resources and services within the community. The Case Manager acts as a liaison between clients, healthcare providers, social service agencies, and other stakeholders to promote effective communication and continuity of care. A critical outcome of this role is to empower clients through advocacy, crisis intervention, and ongoing support, ultimately improving their quality of life and fostering long-term self-sufficiency. The Case Manager also contributes to program development and evaluation to enhance service delivery and address emerging community needs.

Minimum Qualifications:

  • Independent License: LCSWC or LCPC
  • Minimum of two years of experience in case management or social services.
  • Minimum one year of experience working with substance use disorder treatment programs.
  • Strong knowledge of mental health issues, child welfare, and community resources.
  • Excellent communication and organizational skills.

Preferred Qualifications:

  • Certification in Case Management (CCM) or equivalent credential.
  • Experience working in Outpatient Program (OP) and or Intensive Outpatient Programs (IOP).  
  • Experience working with diverse populations and in multidisciplinary teams.
  • Familiarity with program development and evaluation methodologies.
  • Proficiency with electronic health records.

Responsibilities:

  • Conduct comprehensive services for clients enrolled in court ordered substance use disorder treatment program. 
  • Provide Assess client needs for mental, physical and social situations.
  • Advocates support to clients by protecting their rights to medical, financial and legal needs.
  • Work as a bridge between the client, family, doctors insurance companies and other agencies.
  • Monitor ongoing treatment successes and recommend adjustments.
  • Coordinate and monitor services by collaborating with providers, social service agencies, and community resources to ensure clients receive timely and appropriate support.
  • Provide crisis intervention and conflict resolution to address urgent client situations and stabilize their circumstances.
  • Maintain detailed and accurate case records, documenting client progress, service delivery, and outcomes in compliance with organizational and regulatory standards.
  • Participate in program development initiatives by identifying gaps in services and recommending improvements to enhance client outcomes and operational efficiency.

The required skills such as case management, social services, and crisis intervention are essential for assessing client needs and coordinating appropriate care plans effectively. Mental health knowledge and human services expertise enable the Case Manager to understand complex client situations and provide empathetic, informed support.
