How you'll make an impact in this role
Orchestrate the full interdisciplinary care plan from admission through discharge.
Design continuum-of-care plans that integrate patient and family values.
Monitor clinical progress through continuous assessments and record reviews.
Advocate for essential resources and eliminate barriers to patient care.
Serve as a clinical resource for documentation accuracy and post-discharge options.
What minimum qualifications you'll need
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
