Job Summary

We are seeking an experienced Registered Nurse Case Manager for a 13-week travel assignment in Juneau, Alaska.

This role is ideal for an RN with recent hospital case management experience who is skilled in care coordination, discharge planning, utilization review, and interdisciplinary collaboration.

Candidates must have an active Alaska RN license and recent case management experience within the past 2 years.

Key Responsibilities

路聽聽聽聽聽聽聽 Coordinate patient care throughout the hospital stay and transition to the appropriate next level of care.

路聽聽聽聽聽聽聽 Develop and manage individualized discharge plans based on patient needs.

路聽聽聽聽聽聽聽 Collaborate with physicians, nurses, social workers, rehabilitation teams, and other healthcare professionals.

路聽聽聽聽聽聽聽 Assess clinical, psychosocial, financial, and discharge planning needs.

路聽聽聽聽聽聽聽 Identify barriers to discharge and work proactively to resolve them.

路聽聽聽聽聽聽聽 Coordinate post-acute services such as skilled nursing, home health, rehabilitation, durable medical equipment, and outpatient follow-up.

路聽聽聽聽聽聽聽 Communicate with patients and families regarding care plans and discharge expectations.

路聽聽聽聽聽聽聽 Support safe and timely transitions of care.

路聽聽聽聽聽聽聽 Participate in interdisciplinary rounds and care conferences.

路聽聽聽聽聽聽聽 Review patient progress and update care plans as needed.

路聽聽聽聽聽聽聽 Assist with utilization management and medical necessity review as appropriate.

路聽聽聽聽聽聽聽 Coordinate with insurance providers, payers, and external agencies when required.

路聽聽聽聽聽聽聽 Maintain accurate and timely clinical documentation.

路聽聽聽聽聽聽聽 Support compliance with hospital policies, regulatory requirements, and case management standards.

路聽聽聽聽聽聽聽 Advocate for patient needs while promoting appropriate use of healthcare resources.

Required Qualifications

Candidates must have:

路聽聽聽聽聽聽聽 Active and unrestricted Alaska Registered Nurse (RN) license

路聽聽聽聽聽聽聽 Associate Degree in Nursing from an accredited program

路聽聽聽聽聽聽聽 Current BLS certification

路聽聽聽聽聽聽聽 Recent RN Case Management experience within the past 2 years

路聽聽聽聽聽聽聽 Experience working in an acute-care hospital setting

路聽聽聽聽聽聽聽 Strong discharge planning and care coordination skills

路聽聽聽聽聽聽聽 Excellent communication and organizational skills

路聽聽聽聽聽聽聽 Ability to manage multiple patients and competing priorities

路聽聽聽聽聽聽聽 Strong interdisciplinary collaboration skills

Preferred Qualifications

Strong candidates may also have:

路聽聽聽聽聽聽聽 Bachelor of Science in Nursing (BSN)

路聽聽聽聽聽聽聽 Previous travel nursing experience

路聽聽聽聽聽聽聽 Utilization review experience

路聽聽聽聽聽聽聽 Experience with complex discharge planning

路聽聽聽聽聽聽聽 Experience coordinating post-acute services

路聽聽聽聽聽聽聽 Knowledge of Medicare, Medicaid, and commercial insurance processes

路聽聽聽聽聽聽聽 Familiarity with case management software and electronic health records

路聽聽聽聽聽聽聽 Certification in Case Management, such as CCM or ACM

Schedule Requirements

Additional Requirements
