What You’ll Do
- Coordinate and process incoming home health referrals
- Gather and verify patient, insurance, and referral information
- Enter accurate referral information into the electronic medical record (EMR)
- Communicate with hospitals, physicians, facilities, insurance case managers, patients, and team members
- Help coordinate appropriate start-of-care dates
- Follow up on incomplete referrals and help keep the intake process moving
- Manage incoming fax and website communications

- Medical terminology coursework or equivalent education/experience
- Medical office experience required
- Nursing background (CNA, MA, Med Tech) welcomed, but not required
