Riggs Community Health Center is seeking a Clinical Triage Nurse to provide centralized telephone triage, medication refill management, patient portal support, and care coordination in an outpatient primary care setting. This role helps ensure patients receive timely clinical guidance while supporting providers, the Call Center, and nursing teams.

The ideal candidate will be an Indiana-licensed LPN or RN with current CPR certification and strong communication, organization, and critical-thinking skills. Ambulatory care and NextGen EHR experience are preferred.

The Clinical Triage Nurse will assess patient symptoms, provide education and appropriate care recommendations, coordinate follow-up, manage clinical communications, and accurately document patient interactions. The position requires independent clinical judgment, professionalism, confidentiality, and the ability to travel between Riggs locations as needed.
