This role involves coordinating with interdisciplinary teams to ensure that patients receive comprehensive end-of-life care that respects their dignity and comfort while supporting their families through the process. Key Responsibilities: - Conduct comprehensive assessments of patients to identify their physical, emotional, and psychosocial needs. - Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and interdisciplinary team members. - Administer medications and treatments as prescribed, monitoring patients for side effects and efficacy. - Provide education and support to patients and their families regarding the disease process, symptom management, and end-of-life care options. - Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless delivery of care. - Maintain accurate and timely documentation of patient care activities in compliance with regulatory and organizational standards. - Participate in case conferences, team meetings, and quality improvement initiatives to enhance patient care and service delivery. - Offer emotional support and guidance to patients and families, addressing concerns and facilitating effective communication. - Ensure adherence to hospice care policies, procedures, and ethical standards. - Engage in continuous professional development to stay current with best practices in hospice and palliative care.QualificationsRequired Education: - Bachelor of Science in Nursing (BSN) from an accredited institution - Valid Registered Nurse (RN) license in the state of practice Required Experience: - Minimum of two years of clinical nursing experience, preferably in hospice or palliative care settings - Experience in managing patient care plans and coordinating with interdisciplinary teams - Proven track record of providing compassionate care to patients with terminal illnesses Required Skills and Abilities: - Strong assessment and critical thinking skills to evaluate patient conditions and develop appropriate care plans - Excellent communication skills to effectively interact with patients, families, and healthcare team members - Ability to work independently and manage time efficiently in a fast-paced environment - Proficiency in electronic medical record systems and documentation practices - Demonstrated ability to handle emotionally challenging situations with empathy and professionalism - Ability to educate patients and families about disease processes, treatment options, and end-of-life care decisions
