Flexible Territory Options – Choose One We are hiring for two territory options: Territory 1: Athens, Hall, Banks, and Habersham Counties Territory 2: Clarke County Candidates will be considered for either territory based on their geographic area and patient needs.  What You’ll Do As a Hospice RN Case Manager, you will play a key role in ensuring patients receive exceptional, individualized care throughout their hospice journey. Provide compassionate, skilled nursing care to hospice patients in their homes and other care settings Complete comprehensive assessments and develop individualized plans of care Monitor patient conditions and manage symptoms to promote comfort and quality of life Collaborate closely with physicians, hospice clinicians, caregivers, and families Educate and support patients and families throughout the hospice journey Serve as a consistent clinical resource and advocate for your patients Participate in interdisciplinary team meetings and coordinate patient care Maintain accurate and timely clinical documentation Why Bristol Hospice? QualificationsOn An Average Day You Will:  (includes, but not limited to)  Make a true difference—one patient, one family, one compassionate moment at a time.
