Concierge Home Care is looking for a Clinical Care Coordinator, RN to support the clinical team.

This is an opportunity to use your clinical expertise in a Monday–Friday, primarily office-based role while helping clinicians succeed and ensuring patients receive safe, compliant, high-quality home health services.

What You'll Do

• Support and coordinate the day-to-day clinical operations of a home health team
• Provide guidance, support, and coaching to home health clinicians
• Review clinical documentation for Medicare and regulatory compliance
• Monitor quality of care, patient outcomes, and clinical performance
• Collaborate with physicians and interdisciplinary team members
• Help clinicians navigate clinical and documentation challenges
• Coach and mentor clinicians to support professional development
• Ensure agency policies, procedures, and clinical standards are consistently followed
• Help promote a culture of quality, accountability, collaboration, and patient-centered care
• Travel locally as needed to support clinical operations

Compensation & Additional Earning Opportunities

• Competitive salary: $80K-$85K (based on experience)
• Quarterly performance bonus opportunities
• Additional pay-per-visit compensation for eligible field visits completed by the Clinical Care Coordinator

Schedule

• Monday–Friday
• Primarily office-based
• Local travel as needed to support clinical operations
• Leadership and clinical coordination responsibilities without a traditional bedside schedule

Qualifications

• Active, unencumbered Florida RN license
• Minimum 2 years of RN experience
• At least 1 year of skilled home health experience
• Medicare OASIS documentation proficiency
• Strong understanding of home health regulations, clinical documentation, and Medicare compliance
• Strong clinical judgment and communication skills
• Ability to coach, support, and collaborate with clinical professionals
• Valid driver's license
• Reliable transportation and current auto insurance
• Ability to travel locally as needed
