Location: St. Mary Medical Center, Langhorne, PA

Position Purpose:
St. Mary Medical Center, a member of Trinity Health Mid-Atlantic, is looking for a Nurse Navigator to join our Oncology team who serves as a coordinator, counselor, coach and clinician to oncology patients from screening through survivorship.  The Navigator works closely with Physicians and cancer program staff and fellow navigators utilizing an interdisciplinary approach to care.  Expedites the scheduling to keep patients moving quickly along the diagnostic continuum from abnormal imaging to treatment and follow up support services.  Coordinates, participates and maintains cancer program accreditations.  Responsible for providing input to budget development and expense monitoring.  Serves as the primary point of contact along with the medical Director to help build and promote the program.

Responsibilities:

  • Collaborates with physicians and key members of the interdisciplinary team to provide services for the diagnosis and treatment of benign and malignant conditions, from the initial abnormal screening study to treatment, rehabilitation & follow-up care.

  • Works with Manager on budget development and daily expense monitoring and other operational issues as needed.

  • Presents ideas and information in a concise, effective and interpersonally appropriate manner through written and oral forms.

  • Coordinates clinical care related to specific disease issues; helps patients in expediting scheduling of treatment and follow-up care; serves as patient advocate and liaison for coordinating clinical care including diagnostic and surgical consultation appointments.

  • Identifies/researches opportunities for new assessment tools and program development.

  • Acts as liaison for coordinating patient care including office visits, tests and surgical procedures.  Reviews Medication reconciliation with Patient when necessary.

  • Assists patients in maintaining timely appointments for tests, surgery and follow-up care including Radiation Oncology and Medical Oncology and Surgery.

  • Identifies patients for clinical trials/ makes referrals to Research Center as appropriate.

  • Maintains and is responsible for concordance reports.

  • Develops and reviews Survivorship Care Plans at end of treatment with patient.

  • Provides Pre and Post Biopsy education as needed.

Qualifications:

  • BSN

  • Active PA RN License

  • 2+ Years of Nurse Navigator Experience Required

  • Oncology Experience Preferred

  • OCN Certification Preferred
