Competitive Wages & Annual Merit Increase This position will work at 6043 Hollywood Blvd. Schedule is Mon-Fri. 3 days in office, 2 days remote. Duties and Responsibilities: Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services Oversee provision of ECM services and implementation of the Care Plan Offer services where the member lives, seeks care, or finds most easily accessible and withinManaged care plan (MCP) guidelines Connect member to other social services and support needs, such as transportation and housing. Advocate on behalf of members with health care professionals Use motivational interviewing, trauma- informed care, and harm-reduction approaches Coordinate with hospital staff on discharge plan Accompany member to office visits, as needed and according to MCP guidelines Monitor treatment adherence (including medication) Provide health promotion and self-management training Inform patients about other services offered by Saban Community Clinic that may be appropriate for the patient’s family members. Performs other duties as assigned by Supervisor. Position Requirements: Bachelor and/or master’s degree in social work or related fields or equivalent experience 3 years prior case management experience Valid California driver’s license and insurance Must have own transportation Related work experience in public health, health outreach and education Fluency in another language such as Spanish, Russian or Korean preferred.
