Part-Time Home Health RN working with individuals with developmental disabilities.

20 to 30 hours per week.

DUTIES:

Provide weekly assessments of individuals served
Conduct supervisory visits and document results
Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
Review of data tracking and documentation
Staff training and oversight including proxy care
Coordinate medical visits for clients
Medication reconciliations
Other duties as assigned
