Sonic Summary

The role requires early assessment of transitional needs identified during hospitalization or illness.
Candidates must hold a minimum of a Bachelor of Science in Nursing (BSN) and have at least 5 years of clinical experience, including 2 years in case management.
Preferred qualifications include a Master of Science in Nursing (MSN) and 1 year of leadership experience, along with relevant certifications.
The salary for this position is competitive and commensurate with experience.

Manages the care for a specific population; facilitates the safe transition of patient throughout the continuum of care for appropriate utilization of resources, service delivery and compliance with federal and other payer requirements. Provides early assessment of transitional needs identified during their hospitalization, illness, and/or life situation. Performs other duties as assigned.

Education

Minimum: BSN

Preferred: BSN or MSN

Experience:

Minimum: RN with 5 years of clinical experience and 2 years Case Mgmt

Preferred: 1 year leadership experience

Licensure, Registration, Certification

Minimum: RN

Preferred: RN; CCM; ACM
