This role is critical in delivering high-quality care and support to patients, helping them navigate complex health and social issues within the healthcare setting.Key Responsibilities:- Conduct thorough assessments of patients' mental health needs and develop personalized care plans.- Provide individual, family, and group therapy sessions to address mental health issues and promote emotional resilience.- Collaborate with healthcare professionals to integrate social work services into patients' overall care plans.- Advocate for patients' needs and connect them with appropriate community resources and support services.- Maintain detailed and accurate records of patient interactions, treatment plans, and progress notes in compliance with healthcare regulations.- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for effective treatment.- Stay informed about the latest developments in social work practices and mental health therapies to ensure the delivery of evidence-based care.- Offer crisis intervention and support to patients and families during emergencies.- Educate patients and their families about mental health conditions and coping strategies.- Uphold the ethical standards and practices of the social work profession, ensuring confidentiality and respect for all patients.Qualifications: Required Education:- Master's degree in Social Work (MSW) from an accredited institution- Current licensure as a Clinical Social Worker (LCSW) in the state of practiceRequired Experience:- Minimum of 2 years of post-graduate clinical experience in a healthcare setting- Proven track record of working with diverse populations and managing complex cases- Experience in providing therapeutic services, including individual and group therapyRequired Skills and Abilities:- Strong understanding of social work principles, ethics, and best practices- Excellent communication and interpersonal skills for interacting with patients, families, and healthcare professionals- Ability to conduct comprehensive assessments and develop effective treatment plans- Proficiency in electronic health records and documentation processes- Capacity to work independently and collaboratively within a multidisciplinary team- Strong problem-solving skills and ability to manage crisis situations effectively
