What You'll Do

As an Integrated Health Manager (RN), you will:

• Oversee coordination of care and integrated health services across multiple settings.
• Ensure care coordination and integrated health teams maintain up-to-date knowledge and communication with RHD programs.
• Assist leadership with training and development of new staff.
• Ensure enrollment requirements and regulatory standards are met for new individuals served.
• Track physical health measures and ensure information is communicated effectively among care teams.
• Identify program gaps and recommend systems, processes, and policies to improve service delivery.
• Conduct systematic reviews of care plans to ensure individuals are connected to all available resources.
• Provide oversight for case conferences involving individuals with complex needs.
• Collaborate with clinical, mobile, and medical teams to ensure coordinated care.
• Monitor medication refills, laboratory orders, prior authorizations, and medical documentation to ensure timely completion.
• Prepare and communicate quality metrics, statistics, and outcome measures.
• Manage weekly and monthly reporting requirements.
• Supervise assigned staff and support team performance and development.
• Assign and oversee medication-team responsibilities.
• Provide direct nursing support when necessary, including long-acting injectable (LAI) administration, vital signs monitoring, and medication refill coverage.

Qualifications

Required

• Active Registered Nurse (RN) license in the Commonwealth of Pennsylvania.
• 3-5 years of supervisory experience.
• 5-7 years of experience in a behavioral health environment, preferably community-based.
• Valid driver's license and reliable transportation.
• Excellent organizational, communication, and leadership skills.
• Ability to work collaboratively as part of a multidisciplinary team. 

Preferred

• Experience with Electronic Health Records (EHR).
• Experience in integrated care, care coordination, or population health initiatives.
• Knowledge of OMHSAS, DDAP, Medicaid, Medicare, and other regulatory requirements. 

Schedule

• Full-Time
• Hybrid work environment, including office, community, and remote work locations.
• Occasional travel to meetings, trainings, and program sites required.

• Ability to sit, stand, walk, travel, and perform administrative and clinical responsibilities throughout the workday.
• Ability to lift, push, pull, or move up to 50 pounds occasionally.
• Ability to perform essential job functions with or without reasonable accommodation.
