Sunrise provides skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, wound care, and home health aide services throughout Arizona. KEY DELIVERABLES: I am accountable for producing the following results: 1. Assess, evaluate and provide professional services prescribed by the physician to homebound patients 2. Assist the physician in evaluating the patient to determine the type of speech or language disorder and the  appropriate corrective therapy. 3. Develops a rehabilitation plan, based on the evaluation and goals established, in consultation with the physician. 4. Provides rehabilitation services for speech and language disorders. TACTICAL WORK LISTING: List all tactical work for which this position is accountable. 1. Completion of a speech-language pathology curriculum in accordance with state Speech-Language and Hearing Practice  Act, currently licensed to practice as a speech-language pathologist in state, current CPR, auto insurance, state driver license  2. Consistently demonstrates professionalism through appearance, performance and communication. 3. Demonstrates safe competencies to provide care to patients of all ages, respecting patient rights. 4. Ability to regularly complete evaluations, technical treatments, and restorative procedures common in visit-based speech  therapy practice. 5. Observes and records the patient’s reaction to treatment and changes in the patient’s condition. Notifies physician with   any changes and appropriate health professionals. 6. Maintains confidentiality of patient assessments and medical records. 7. Demonstrates the ability to communicate effectively with the patient/family. 8. Demonstrates the ability to communicate effectively with other members of the health care team and Agency staff. 9. Attend Case Conference and makes referrals to other disciplines, as indicated by the needs of the patient. 10. Consistently adheres to standard precautions, transmission precautions, aseptic techniques and infection control   Guidelines 11. Instructs health care team members and the family in methods of assisting the patient to improve means of   communication in regard to speech and language. STANDARDS: List quantify, quality and behavior standards for which this position is accountable. Position Specific: 1. Communicate with RN/Therapist on a regular basis 2. Meet with Director of Patient Services on a regular basis. 3. Attend Case Conference and other required inservices as required. Qualifications
