Position Responsibilities:

聽聽聽聽聽聽 Under the direction of the physician he/she will be assigned a panel of high-risk geriatric participants to oversee and medically manage.

聽聽聽聽聽聽 In collaboration with a physician, conduct initial, semi-annual and unscheduled assessments and reassessments per policy and regulations and perform additional physical assessments of participants as needed.

聽聽聽聽聽聽 Assist in the daily oversight of the Eldercare Clinic. Including clinic nurses, home care and UR nurse, medical assistants and other technicians..

聽聽聽聽聽聽 Perform rounds in Eldercare contracted nursing homes, hospitals and other care sites as needed.

聽聽聽聽聽聽 Conduct home visits as needed for medical interventions.

聽聽聽聽聽聽 Participate in the development and implementation of participant plans of care as part of and in collaboration with the interdisciplinary team.

聽聽聽聽聽聽 Function as a member of the interdisciplinary team.聽 Maintains regular attendance at, and participate in interdisciplinary team meetings; communicate participant changes, collaborate on care planning decisions and coordination of 24 hour care delivery.

聽聽聽聽聽聽 Integrate the primary care treatment plan into the overall plan of care developed in collaboration with IDT (Interdisciplinary Team)

聽聽聽聽聽聽 Interact with team members to meet emergent and acute needs of participants..

聽聽聽聽聽聽 Reviews all hospital, ED or other acute settings admission information and assists the IDT team with the discharge plan and services needed to ensure a safe transition from one level of care to another.

聽聽聽聽聽聽 Provide preventive health care and maintenance for participants, including immunizations, screenings and monitoring of pertinent indicators. Follow the Eldercare鈥檚 clinical protocols

聽聽聽聽聽聽 Prudently prescribe medications, therapies and other treatments for participants.

聽聽聽聽聽聽 Demonstrates advance knowledge of the general diagnostic and therapeutic procedures for the at risk participants of the Eldercare PACE program

聽聽聽聽聽聽 Interviews patients and completes a full comprehensive medical history and physical examination within PACE regulations and Eldercare policies.

Qualifications:

聽聽聽聽聽聽 State of Florida Advanced Practice Registered Nurse License; ACLS Certification

聽聽聽聽聽聽 Advanced Practice Registered Nurse, Geriatric Certification preferred. Master of Science in Nursing

聽聽聽聽聽聽 At least 1 years experience as an APRN (preferable), primary care or other medical specialty preferable geriatrics experience preferred, 5 years experience as a RN and 1 year working with the frail or elderly
