We are committed to connecting and serving our clients where they live.  Position’s Role Summary:North Texas Behavioral Health Authority (NTBHA), based in Dallas, is responsible for developing a collaborative system of care for children, adolescents, and adults with severe mental illness, serious emotional disturbances, and substance use disorders. This position’s responsibilities must align with NTBHA values, goals, regulations, policies, procedures, and all applicable documentation standards and regulatory requirements. This position will be based out of the Heights Domestic Violence Shelter, helping to screen individuals that present with potential mental health concerns.The Care Coordinator position provides timely intervention at the least restrictive level of care through comprehensive, trauma-informed, person-centered, recovery- and resilience-oriented services to help individuals prevent decompensation and relapse. This role primarily serves an assigned county and requires strong organizational and clinical skills to ensure continuity of care and provide referral and resource solutions across intersystem and interdepartmental processes within NTBHA and community stakeholders.The Care Coordinator provides complex services including screenings and assessments, crisis follow-up, and transitional services that effectively connect individuals with mental illness and/or co-occurring disorders to appropriate care. These efforts help prevent re-emerging crises and address barriers to treatment.Care Coordinators actively participate in staffing's to determine appropriate treatment options, and collaborate effectively with internal departments, law enforcement, state and local hospitals, and community agencies and organizations. This position also provides skills training and case management focused on reducing risks related to recidivism, hospitalization, and/or homelessness.This position specifically requires availability on non-traditional days and hours. The role may also require after-hours cellular availability and flexibility to work at other times and some weekends. This position requires minimal travel with an expectation of reliable transportation, plus a valid driver’s license and car insurance required. This position requires at least one shift per month doing virtual jail assessor screenings.Travel is required as part of this role’s duties.   Travel can vary from 20% to 100% per day. This would include all counties within NTBHA Catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties). This role may be required to use a fleet vehicle and/or a personal vehicle for business needs.  This may include, but is not limited to field & home visits, offsite meetings, co-located sites and/or pick up items for business needs, etc.  If your program requires transport of individuals, this would only be allowed with a company vehicle and approval from management. This position is classified as exempt and requires flexibility in scheduling to meet organizational needs, including adjusting work hours as necessary to ensure job responsibilities are completed. Qualifications: Qualifications required:  Bachelor’s degree in psychology, social work, or a related behavioralhealth field; a master’s degree is preferred. One or more years of direct care experience working with individuals with serious mental illness and/or substance use disorders. Preference given to experience in psychiatric crisis service settings. Experience participating as part of a treatment team that develops and monitors treatment plans for individuals with chronic and serious mental illness. Current knowledge of community resources, providers, and social supports for homeless and indigent populations within the NTBHA service area. Understanding of and experience with HHSC programming and systems preferred, including Texas Resilience and Recovery, CMBHS, and CARE. Must possess and maintain a valid driver’s license and maintain a clear background record as required for healthcare organizations under state and federal contracts. Job duties, responsibilities, and skills:  Reports to the Program Manager or Team Lead and performs daily, weekly, and monthly tasks supporting referrals, documentation, and reporting. Works autonomously and makes decisions consistent with applicable rules, regulations, policies, and procedures. Creates, analyzes, prepares, and maintains records, files, documents, reports, and correspondence. Identifies problems, facilitates solutions, and demonstrates knowledge of community resources, local indigent behavioral health systems, service coordination principles, standards, and program policies and procedures. Communicates effectively and uses strong interpersonal skills to convey ideas and complex concepts to individuals from diverse cultural and socioeconomic backgrounds, both orally and in writing. Works constructively and collaboratively with other professionals and staff, community representatives, individuals with disabilities, and family members. Fosters positive, effective working relationships with inter-agency and intra-agency team members, encouraging collaboration across organizational lines. Respects diverse viewpoints and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance for all employees, patients, and visitors. Provides trauma-informed, resilience-oriented care with cultural humility. Assesses client needs and links them appropriately with community and provider resources. Must be able to work on and have knowledge of a PC (personal computer). Possesses advanced Microsoft Office skills, including Outlook, Word, Excel, and PowerPoint, with the ability to learn company-specific applications and software.
