RN Case Manager

A leading home health organization in Portland, Oregon is seeking an RN Case Manager to join its growing clinical team! 

Pay: $55.00 - $60.00/hour

RN Case Manager Opportunity: 

• Full-time, permanent, direct-hire position 
  
• Monday-Friday, 8:00 AM - 5:00 PM schedule with weekend rotations 
  
• Guaranteed weekly pay 
  
• Mileage reimbursement provided

RN Case Manager Responsibilities: 

• Complete initial patient assessments and develop comprehensive plans of care 
  
• Regularly evaluate patient conditions and revise care plans as needed 
  
• Utilize assessment findings to establish nursing diagnoses and treatment goals 
  
• Administer medications and treatments as prescribed by physicians 
  
• Educate patients and families regarding care plans, treatments, and health management 
  
• Coordinate discharge planning and ensure continuity of care 
  

RN Case Manager Qualifications: 

• Current Oregon Registered Nurse (RN) license required 
  
• Minimum of one year of recent acute care experience required 
  
• Current BLS/CPR certification required 
  
• Valid driver's license and auto insurance required 
  
• Associate's or Bachelor's Degree in Nursing required 
  
• Home health experience preferred
