Case Manager/Care Coordinator Level 2 – School-Based Services

Southern Highlands Community Mental Health Center is seeking a full-time Case Manager/Care Coordinator Level 2 to join our School-Based Services team in Wyoming County.

This position works with children, adolescents, and their families to help connect them with essential behavioral, medical, social, educational, and health-related supports. The Case Manager/Care Coordinator works closely with families, school personnel, and service providers to coordinate care, advocate for students, and support the development and implementation of person- and family-centered treatment plans.

Key Responsibilities:

• Assist children, adolescents, and families in accessing needed behavioral, medical, social, educational, and health-related services and supports.
• Coordinate care with families, school personnel, providers, and other involved supports.
• Advocate for the needs of students and their families.
• Assist with the development and implementation of person- and family-centered treatment plans.
• Help connect students and families with appropriate resources and services.

Education & Experience Requirements:

Applicants must meet one of the following qualifications:

• Bachelor’s or Master’s degree in a human services-related field, per West Virginia Board of Social Work requirements; OR
• Licensed Psychologist with a Master’s or Doctoral degree from an accredited program; OR
• Licensed Registered Nurse.
