Job DetailsJob Location: New Hartford, NY 13413Position Type: Per DiemEducation Level: 2 Year DegreeSalary Range: $40.00 - $55.00 HourlyDEPARTMENT:        Nursing   POSITION:               RN Shift Supervisor (SNF)   SUPERVISOR:         Director of Nursing Services   HOURS:                   Per diem, one weekend a month, 2 days out of the month, 2 holidays a year.                               SHIFTS:                      3:00 PM - 11:00 PM: $40.00 - $50.00 per hour, commensurate with experience.                                     11:00 PM - 7:00 AM: $45.00 - $55.00 per hour, commensurate with experience.    Responsible for the supervision and daily operation of the nursing houses during scheduled shift.  Duties in this area include overseeing of staff to provide adequate nursing care, to document accurately, and to insure physician's orders are fol­lowed and informs families with significant changes in condition. Responsible for the staffing coverage of individual shift and to provide oncoming shift with staffing coverage, when necessary, to maintain prescribed levels. Assumes ultimate responsibility for all resident care delivered during duty hours. Responsible for ancillary personnel performance in the absence of their department head and intervenes when problems arise. Assures nursing staff implements timely documentation of changes in residents' physical and/or emotional conditions in the resi­dent record and/or care plan as necessary and MD notification. Assesses sudden or drastic changes in residents' condition and initiates directive for immediate care with appropriate documentation of assessments. Assures proper communication of resident related information to the following shift. Completes job performance evaluations for licensed personnel assigned to shift and provides guidance and suggestions to other staff members responsible for evaluations as required. Assists the DNS in planning and directing the Nursing Department including interpretation and administration of department poli­cies and procedures. Monitors nursing staff to insure that policies and procedures are followed as written, i.e. monitors nursing staff for the proper administration of medications and treatment. Responsible for delivering educational programs to staff members as programmed by the Education Department staff, main­taining attendance records for same and continuing orientation of new employees assigned to that shift as necessary. Responsible to assign staff members to inservice classes and continuing education programs. Assists licensed staff with notation of physician orders and interprets transcription as necessary. Determines need for notifications of physician, families and/or administration of events occurring on shift. Actively participates in nursing administrative meetings. Writes investigative reports on all incidents involving significant injuries per facility policy. Submits quarterly meeting minutes to Director of Nursing Services per request. Access to the entire medical record of all patients/residents. Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.  Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties. Performs any other duties assigned by Director of Nursing Servic­es.  QualificationsGraduate of an approved/accredited School of Nursing and current­ly licensed as a Registered Professional Nurse in New York State. Previous leadership experience with at least one-year supervisory experience in the field of nursing. Active interest in gerontological and rehabilitative nursing. One year geriatric nursing preferred. Displays an active interest in pursuing continuing education to keep knowledge and skills current. Knowledge of New York State codes. Demonstrates competency by completing an annual job specific skills checklist. Demonstrates proficiency in nursing skills and leadership ability.     Additional Information: Charles T.
