Job DetailsJob Location: 211 Canal Street - Staten Island, NY 10304Position Type: Full TimeEducation Level: High School/GEDSalary Range: $22.50 - $24.73 HourlyJob Shift: DaySUMMARY: The Case Manager will work in our Hopwa Family/Hopwa Singles program and be responsible for ensuring client safety and providing care coordination and case management services in an effective and efficient manner for all clients. Work Schedule: Full time, Monday - Friday 9:00 a.m. - 5:00 p.m. RESPONSIBILITIES:  Provide supportive counseling to clients and their families Healthcare Advocacy for clients with their Infectious Disease Provider, Psychiatrist, Mental Health providers as well as providing escort to appointments Facilitate and attend case conferences with other service providers Conduct assessment, reassessment and service plans tailored to meeting client’s needs, providing appropriate referrals, advocacy and home visits Attend and assist in preparation of Team Meetings and Community Meetings Provide education to clients on healthcare promotion and individual living skills Provide monthly apartment inspections and document in progress notes QualificationsQUALIFICATIONS NEEDED: High School/GED Required. Fluent written and oral communication in English required, Spanish preferred. One to two years of case management/social services background is needed. Knowledge regarding housing services and entitlements is needed. A clean, valid Driver’s License is required. Must have excellent communication skills, both orally and in writing, with individuals and groups regarding complex and sensitive issues.
