Job DetailsJob Location: HUD Permanent Supportive Housing - Staten Island, NY 10310Position Type: Full TimeSalary Range: $23.63 - $31.87 HourlyJob Shift: DayWork Schedule: Monday - Friday, 9:00 a.m. - 5:00 p.m., with some flexibility needed Summary: The Case Manager provides in-home and on-site case management services to promote a healthy lifestyle and positive behavior changes in our HUD program. RESPONSIBILITIES: Oversees housing units and provides case management services to tenants within the program. Completes intakes, along with pre/post-placement assessments and re-assessments, as required. Develops service plans in conjunction with team members and tenants. Provide services related to transitional living; including but not limited to meeting with tenants, apartment inspections and management, good neighbor relation skills, assessments/reassessments of needs, and coordination with medical/mental health providers. Assists tenants in accessing nutritious food, as well as in obtaining and maintaining entitlements, vocational training, and job placement, as possible. Provides ongoing coordination with primary care physicians, mental health, and other providers. Responds to tenants requests for information and referrals. Maintenance of clients’ paper and electronic charts. Completes required documentation on a timely basis. Attend interdisciplinary meetings, staff trainings, and supervisions. QualificationsPosition requires a Bachelor’s degree in social work or related field. Prior working experience with the homeless population and housing disadvantaged population a plus. Must have a clean and valid driver’s license. The ability to establish workload priorities and balance diverse duties is needed. The case manager will need to have exceptional oral and written communication skills. They must possess high degree of skill in working with individuals and groups over a broad range of topics and issues, many of which require extreme sensitivity and insight. In addition, the case manager shall perform any duties as required to meet the needs of the program and the clients.
