Job DetailsJob Location: GRCC NORC - New York, NY 10025Salary Range: $66,300.00 - $70,000.00 SalaryThe Aging Services Program Manager will 路 Be responsible for providing coordination of services and outreach to older adults who are registered to Isaacs Center programs. 路 Provide support in maintaining behavioral and operational challenges that arise. 路 Develop, maintain, and continually update a schedule of programs and supportive services in alignment with the vision and contractual obligations for our NYC Aging Contracts 路 Serve as liaison between specialists, consultants, volunteers, interns, supplemental service providers, property management, and other community stakeholders. 路 Conduct outreach to share program services and offerings to new and potential members. 路 Supervise program support staff. 路 Ensure compliance with NYC Aging, NYSOFA, and Internal requirements as needed. 路 Work closely with NORC Program Manager to create programming opportunities and increase 聽 Salary: $66,300- $70,000 聽 Roles, Responsibilities, and Essential Duties Roles, Responsibilities, and Essential Duties Ensure participants gain access to social services, wellness education, arts and culture, and recreation activities Create and manage a monthly calendar of activities, services, and events Develop and maintain relationships with community partners and consultants Develop professional provider relationships with older adults, families and caretakers Develop individualized service plans and reassess regularly Develop and maintain a directory of supportive services and agencies available locally Organize and lead community center trips and outings 路 Assist with outreach efforts and follow-up to referrals Advocate for clients' rights, entitlements, and benefits 路 Communicate with hospital and rehabilitation social workers when needed Submit weekly Dashboard and Service Log reports and maintain documentation Work with supervisor and government agencies to ensure compliance Provide support for programs through planning, implementation, management, and evaluation 聽 Qualifications: The Aging Services Program Manager must possess strong writing, speaking, interpersonal, and organizational skills. It is preferred that the candidate hold a bachelor's degree. The candidate must have demonstrated experience providing group facilitation, maintaining strong interpersonal skills, and managing data and outcomes. The candidate must have a history of working with diverse and high-needs aging populations, value team building, and support the development of a community to address complex issues. 路 Bachelor's degree 路 Prior experience working with the elderly highly preferred 路 Bilingual, English/Chinese or English/Spanish highly preferred 路 Ability to handle confidential information with discretion. 路 Ability to work some flexible hours 路 Working knowledge of community resources and service providers to older adults 路 Ability to work effectively with cultural, economically and educationally diverse populations 路 Strong verbal, written and interpersonal communication skills. 路 Demonstrated knowledge of aging-related issues. 路 Computer literacy includes Microsoft Word, Excel, and Outlook
