Job DetailsJob Location: 66 Boerum Place, 66 Boerum Place, Brooklyn, NY, 11201 - Brooklyn, NY 11201Position Type: Full-TimeEducation Level: Bachelors Degree or EquivalentSalary Range: $31.82 - $31.82 HourlyTravel Percentage: Up to 75%Job Shift: DayThe Socio Therapist provides trauma-informed therapeutic case management and support to children, youth, and families involved in foster care. This role partners with families to develop individualized service plans, promotes safety and permanency, provides crisis intervention, coordinates care with internal and community providers, implements evidence-based practices, and advocates for youth through court appearances, family meetings, and ongoing case management. Work Schedule: This is a hybrid position, requiring a combination of in-office, community-based, and remote work, based on program needs. Primary Responsibilities and Essential Functions of Position: Creates, in partnership with families, plans which address safety, behavioral, psychological and permanent needs.      Monitors safety, well-being, and permanency progress by conducting regular casework contacts and providing the necessary services to meet the needs of the children and families.      Complete timely documentation of all reports and casework activities including FASPS, Psychosocial, Adoption Home studies, Kinship Home studies, court reports      Actively participates in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.      Provide crisis intervention as needed      Implement evidence-based models with foster parents, parents, discharge resources and youth.      Coordinate care with other agency and community-based programs such as Behavioral Health & Wellness and Health Homes Provide family planning counseling to youth and document discussion with foster parents      Work to identify family resources and support for youth through Family Finding evidence-based practice.      While performing the responsibilities of the job, the employee is required to frequently communicate with clients who have inquiries about services that you are providing. Must be able to exchange accurate information in these situations   Qualifications Bachelor's Degree required; BSW preferred.  Experience supporting families and adolescents with social services. Must have a commitment to work from a strength-based, youth development and trauma informed perspective. Ability to work with diverse population of clients and team members Must be highly organized with ability to meet program deliverables and deadlines Must have excellent verbal and written communication skills Ability to work flexible hours - some evenings or early mornings as needed to meet client needs Ability to use public transportation to travel throughout New York City or Agency vehicle when available.
