Responsibilities

  • Evaluate & Treat: Conduct initial and periodic assessments for speech, language, and swallowing disorders.
  • Goal Setting: Develop and adjust measurable treatment plans based on individual patient needs and physician orders.
  • Collaborate: Partner with the Rehabilitation Therapist and nursing staff on care transitions and discharge planning.
  • Mentor: Provide clinical guidance to new staff, students, and volunteers. 

Qualifications

  • Licensure: Current New York State SLP License and registration NYS Office of the Professions.
  • Required Experience: Minimum 1 year in a nursing home; rehabilitation setting; Patient Driven Payment Model (PDPM) experience required 
  • Preferred Experience: experience with pediatric patients
  • Education: Master’s Degree in Speech-Language Pathology from an accredited program. 

Physical Requirements

  • Ability to move throughout the facility and stand/sit for extended periods.
  • Capability to lift or position up to 30 lbs occasionally.
  • Acute sensory skills to assess patient vocalizations and non-verbal cues.
