Responsibilities
- Evaluate & Treat: Conduct initial and periodic assessments for speech, language, and swallowing disorders.
- Goal Setting: Develop and adjust measurable treatment plans based on individual patient needs and physician orders.
- Collaborate: Partner with the Rehabilitation Therapist and nursing staff on care transitions and discharge planning.
- Mentor: Provide clinical guidance to new staff, students, and volunteers.
Qualifications
- Licensure: Current New York State SLP License and registration NYS Office of the Professions.
- Required Experience: Minimum 1 year in a nursing home; rehabilitation setting; Patient Driven Payment Model (PDPM) experience required
- Preferred Experience: experience with pediatric patients
- Education: Master’s Degree in Speech-Language Pathology from an accredited program.
Physical Requirements
- Ability to move throughout the facility and stand/sit for extended periods.
- Capability to lift or position up to 30 lbs occasionally.
- Acute sensory skills to assess patient vocalizations and non-verbal cues.
