Job Details: Job Location: HealthPoint Family Care Newport and Administration - Newport, KY, Salary Range: $135000.00 - $142000.00Salary/year, HealthPoint is in search of a full time Family Nurse Practitioner for our Northern Kentucky locations.HealthPoint is an approved loan forgiveness site! How to apply for loan forgivenessBase: $135,000-$142,000 (based on years of experienceAnnual Quality Bonus Potential: $23,000Monthly production bonus: unlimited $10 for every visit over 263 per monthCombined average annual salary:$160,000Hours-40 hours per week, rotating schedule 3-5 days per week and 2 evening per week and 1-2 Saturdays per month.(some months will be 1 Saturday and some will be 2 Saturdays)Paid Time Off:  30 Days of PTOHolidays: Up to nine Paid Holidays and as well as your Birthday off paid.Years of Service Award: Continuous Years of Service bonus. Starting at year 2 with $4,500 with progressive increases. 401k Plan: $1 for $1 match on first 3% of income and $0.50 on the $1 for 4th and 5th% for a max match at 4% of your income.
