Job Purpose:
To plan and coordinator bereavement services for PruittHealth Hospice families and patients
•           Previous training and/or experience in death and loss issues.Â
•           Ability to interact with a variety of different age groups.      Â
•           Ability to work well independently and within an interdisciplinary team setting.           Â
•           Ability to communicate effectively orally and in writing with individuals and groups.  Â
•           Ability to handle confidential matters.  Â
•           Ability to work a flexible schedule.                      Â
•           Possess knowledge of computer software, including proficiency in word-processing.           Â
Other Division or Job-Specific Essential Administrative Functions:
•           Plan, organize, implement and monitor the PruittHealth Hospice bereavement care.                     Â
•           Supervise bereavement care volunteers and develop volunteer inservice trainings.   Â
•           Develop a bereavement plan of care for PruittHealth Hospice families.                         Â
•           Review bereavement assessment.         Â
•           Refer families to other community resources as appropriate.Â
•           Organize and facilitate bereavement counseling groups.       Â
•           Serve as a bereavement representative at PruittHealth Hospice interdisciplinary team meetings.
•           Document interventions with patients and families and update bereavement care plans accordingly.     Â
•           Present training sessions on grief and loss to new hospice volunteers as part of the overall volunteer training program.      Â
•           Assist in conducting educational workshops for the community.                    Â
•           Assist in providing bereavement support to staff members of facilities where hospice patients reside. Â
•           Coordinate with the Director of Health Services (DHS) and the interdisciplinary team that the needs of patients and families are being met related to bereavement.                        Â
•           Other duties as assigned by the Supervisor and/or the Administrator.      Â
MINIMUM EDUCATION REQUIRED:
•           Completion of a Master’s degree from an accredited theological school/seminary.
•           Completion of 1-4 units in Clinical Pastoral Education from a nationally recognized provider.
Â
MINIMUM EXPERIENCE REQUIRED:
•           Two years of ministerial experience.
•           Appropriate endorsement within his/her denomination (e.g., ordination).
•           Previous training and/or experience in death and loss issues.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
•           Current Driver’s License and Automobile Liability Insurance
