Position Summary

SmithLife Homecare is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. In this flexible role, you will provide skilled nursing services to clients in their homes, supporting safe, high-quality care and promoting independence.

Working under the supervision of a Registered Nurse (RN), you will implement individualized care plans, monitor client conditions, and support families throughout the care journey.

Key Responsibilities

  • Deliver skilled nursing care in accordance with the established Plan of Care 
  • Administer medications, injections, and treatments as prescribed 
  • Monitor and document client condition, including vital signs and response to care 
  • Observe and report changes in physical, emotional, or cognitive status 
  • Assist with clinical procedures including wound care and infection prevention 
  • Support clients with activities of daily living as needed 
  • Educate clients and families on care needs and disease management 
  • Maintain accurate, timely documentation in accordance with agency standards 
  • Collaborate with RNs, care coordinators, and interdisciplinary team members 
  • Follow all infection control protocols and agency policies 

Qualifications

  • Active LPN license in Maryland and/or Washington, DC (in good standing) 
  • Graduate of an accredited practical nursing program 
  • Minimum 3 years of clinical nursing experience preferred 
  • Strong clinical judgment, communication, and documentation skills 
  • Experience in home care, geriatrics, or chronic disease management preferred 
  • Ability to work independently and adapt in a home-based setting 

Physical Requirements

  • Ability to lift, assist, and reposition clients as needed (50+ lbs) 
  • Frequent standing, walking, bending, and kneeling 
  • Full use of hands and arms for clinical tasks 

Schedule & Status

  • PRN / Per Diem (As Needed)
  • Variable hours based on client needs, including potential evenings, weekends, or short-term assignments 

Compensation

  • Competitive hourly rate based on experience 
  • Opportunities for additional hours based on performance and availability
