We are seeking a skilled and empathetic Physical Therapist to join our team. In this role, you will be responsible for providing individualized rehabilitative care to our patients in a home health setting. Your expertise will help patients regain independence, reduce pain, and improve mobility, ultimately enhancing their quality of life.
Responsibilities
• Patient Assessments: Conduct thorough evaluations of patients’ physical abilities, medical history, and limitations to create personalized treatment plans.• Treatment Plans: Develop individualized rehab programs based on assessments and patients’ goals, including therapeutic exercises, manual techniques, gait training, and modalities.• Patient Care: Provide hands-on interventions to enhance mobility, strength, and flexibility, while monitoring progress and adjusting treatment as necessary.• Family & Caregiver Support: Educate patients and their families on exercises, techniques, and injury prevention strategies to continue rehabilitation outside of sessions.• Documentation: Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and outcomes.• Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care and recovery.• Discharge & Follow-Up: Discharge patients when goals are met, providing recommendations for continued care or referrals as needed.• Other Duties: Perform other related tasks as assigned, ensuring flexibility in patient care as necessary.
Qualifications
• Qualifications: Licensed Physical Therapist with a valid state license.• Experience: Previous experience in a clinical or home health setting, especially with geriatric care, is preferred.• Skills: Strong assessment, communication, and organizational skills.• Patient-Centered Care: Empathy and the ability to work with patients facing various physical challenges.• Independence: Ability to work independently with a flexible schedule.• Tech Proficiency: Experience with electronic medical records (EMR) and Microsoft Office Suite.• Other: Must pass a criminal background check and drug screen.

Pay Range
