We are currently hiring a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Leesburg and surrounding areas.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans
• Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices
• Monitor patient progress and adjust treatment plans as needed
• Complete timely and accurate clinical documentation
• Communicate with physicians, nurses, and the interdisciplinary care team
• Supervise PTAs as required

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Florida PT license in good standing
• Minimum 1 year of PT experience preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements
