Sunplex Subacute Center is looking for a MDS Coordinator to join our growing team!

Fulltime | 8am to 4:30pm, Mon-Friday

MDS Coordinator duties include but are not limited to:

Maintain and update schedules of MDS's that are due
Coordinate the work of the Interdisciplinary Team to complete the MDS, which includes the CAAs and care plan
Enter or supervise input of MDS into computer systems
Act as the leader of the MDS and care plan meetings
Coordinate the completion of the care plan
Other tasks as required

Requirements

Registered Nurse ( RN ) license in good standing required
