We are seeking a creative, community-minded Population Health Educator to join our team. In this dynamic role, you won’t just sit behind a desk—you will actively engage patients through clinic co-visits, group education, and community outreach to break down health barriers and improve long-term wellness outcomes.

What You’ll Do (Main Duties & Responsibilities)

1. Patient Education & Engagement

• Provide Interventions Across Settings: Lead engaging, low-literacy health education through one-on-one sessions in the clinic, group classes, and virtual sessions.

• Clinical Co-Visits: Work side-by-side with providers during clinic visits, setting self-management goals with patients and documenting care plans in the Electronic Health Record (EHR).

• Basic Screenings: Perform routine preventive checks within scope, including blood pressure re-checks, diabetic foot inspections, height/weight measurements, and standardized SDOH/health risk screeners.  

2. Health Equity & Community Outreach

• Build Local Partnerships: Cultivate relationships with community groups and regional organizations to expand resources and increase healthcare access for rural residents.

• Design Inclusive Materials: Collaborate with Marketing and Quality Improvement Teams to evaluate, recommend, and create culturally appropriate, low-literacy, multi-lingual educational resources (print and audio/visual).  

• Community Needs: Participate in community health needs assessments to identify emerging local needs and tailor programming accordingly.  

3. Population Health & Teamwork

• Serve as an internal consultant to providers and clinic staff regarding patient educational needs.  

• Collaborate with the Population Health Team to improve clinical quality metrics (e.g., hypertension control, diabetes management, preventive screenings).

• Maintain accurate records of class attendance, outreach activities, and patient encounters.

• Effective Communicator: Ability to explain complex medical concepts using clear, low-literacy, and culturally tailored communication.

• Clinical Competency: Comfort performing basic clinical tasks (blood pressure checks, screening tools, basic first aid).  

• Community Connector: Skilled in building trust across diverse populations, including agricultural workers, low-income families, and non-English speaking patients.

• Adaptable & Collaborative: Comfortable in clinic exam rooms, community centers, and virtual spaces alike.

Preferred Qualifications:

• Bilingual in English/Spanish (strongly preferred).

• Prior experience in a Community Health Center (FQHC), public health, or rural health setting.

• Certification as a Community Health Worker (CHW) or Health Education Specialist (CHES) is a plus.
