We are looking for a compassionate, organized, and resourceful person to join the team as an Outreach & Navigation Specialist. This position is often one of the first points of contact for individuals and families seeking help. You will listen to their needs, help determine eligibility, connect them with resources, and support them as they work toward greater stability and self-sufficiency.
This is a good opportunity for someone who enjoys helping people, building relationships, solving problems, and working as part of a nonprofit human services team.
What You’ll Do
As an Outreach & Navigation Specialist, you will:
- Respond to calls and requests from individuals and families seeking assistance.
- Conduct intake appointments, assessments, and eligibility screenings.
- Help participants gather documentation and understand program requirements and next steps.
- Identify household needs, strengths, barriers, and available resources.
- Work with participants to develop meaningful goals and action plans.
- Connect participants with EightCAP programs and community services.
- Provide follow-up support and help participants navigate challenges.
- Offer basic budgeting assistance, financial education, and other assigned educational services.
- Participate in community outreach events and represent EightCAP throughout the service area.
- Build relationships with community partners and referral sources.
- Maintain accurate case notes, service records, applications, and data-entry records.
- Protect participant confidentiality and follow program documentation requirements.
What We’re Looking For
The successful candidate will have:
- An associate’s degree in human services, social work, public administration, community development, business administration, or a related field.
- A bachelor’s degree in a related field is preferred.
- An equivalent combination of relevant education and experience may be considered.
- Experience working with individuals and families in a helping profession is preferred.
- Strong communication, interpersonal, organizational, and computer skills.
- The ability to build respectful relationships with people from different backgrounds and life experiences.
- The ability to work independently while contributing to a collaborative team.
- Community Health Worker certification or the ability to obtain certification within six months.
- A valid Michigan driver’s license and reliable transportation for travel throughout EightCAP’s service area.
Candidates must have a bachelor’s degree or higher in a field related to the position to be eligible for placement at the HS04 level.
What We Value
At EightCAP, our work is grounded in four core values:
- Accountability: Following through, maintaining accurate records, and taking ownership of participant needs.
- Communication: Listening first, explaining information clearly, and keeping participants and partners informed.

- Health, dental, and vision insurance
- Life and AD&D insurance
- Medical flexible spending benefits
- A 401(k) retirement plan with a 5% employer match beginning on your one-year anniversary
