Job Details: Job Location: Ottumwa Office - Ottumwa, IA, Position Type: Full-Time/Part-Time, Education Level: 4 Year Degree, Salary Range: $28.00 - $36.00Hourly, Job Shift: Day, Are you an RN or LPN who enjoys providing hands-on nursing care and building a strong connection with the patients you serve?Iowa Home Care is looking for a dependable and compassionate Shift Nurse to provide skilled nursing care to patients in their homes throughout the Ottumwa area. This role is ideal for a nurse who enjoys working closely with one patient and family at a time and takes pride in providing consistent, attentive care throughout an assigned shift.Rather than moving between multiple patients in a facility, you’ll have the opportunity to become familiar with your patient's routines, preferences, needs, and goals while helping them remain safe and comfortable at home.What You'll DoAs a Home Health Shift Nurse, you will provide direct nursing care throughout your scheduled shift while following the patient's individualized plan of care.Responsibilities may include: Provide skilled nursing services in the patient's home during scheduled shifts Follow the established plan of care and physician orders Monitor the patient's condition throughout the shift and recognize changes that may require intervention Administer medications and treatments as ordered and within your scope of practice Perform nursing procedures appropriate to the patient's needs and level of licensure Monitor and document vital signs, symptoms, intake/output, and other relevant clinical information Assist with respiratory, nutritional, mobility, and other skilled care needs as applicable to the patient Provide appropriate safety, infection prevention, and emergency interventions Communicate changes in patient condition promptly to the appropriate RN, physician, supervisor, or care team member Maintain accurate and timely nursing documentation Assist patients with maintaining comfort, dignity, and as much independence as possible Provide education and support to family members and caregivers Build a trusting relationship with the patient and family while maintaining professional boundaries Work collaboratively with the patient's broader healthcare team Follow agency policies, clinical standards, and applicable state and federal regulations What Makes Shift Nursing Different?Focused Patient CareInstead of caring for multiple patients at once, you'll have dedicated time to focus on one patient's individual needs during your shift.Meaningful RelationshipsWorking consistently with the same patient allows you to learn their routines, recognize subtle changes, and become a trusted member of their care team.Clinical IndependenceYou'll use your nursing knowledge and assessment skills throughout your shift while having support from Iowa Home Care's clinical leadership when you need it.Care Where Patients Need It MostYour work takes place in the patient's home, where your nursing expertise can help them remain safe, comfortable, and connected to their normal routine.What We OfferWe want our nurses to feel supported both in the field and as members of our team. One (1) to two (2) years of recent acute care experience in an institutional setting. Current RN licensure  and CPR certification. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
