PACE APP

Reid Health is seeking apatient-centered and compassionate NP or PA to join our PACE (Program of All-Inclusive Care for the Elderly) team. This APP will provide primary medical management for PACE participants in collaboration with our PACE physicians and interdisciplinary care team, with a focus on comprehensive assessments, ongoing care planning, and supporting the health and well-being of our participants.

Position Details:

• Team: 1 physician and 1 NP
• Office Hours: 8am to 4:30pm

Education/Experience:

Education Requirements:  Successful completion from a qualified NP/PA program

Licensure: Indiana Professional License

Certifications: Must maintain BLS/ACLS
