Description

Nursing that goes beyond the bedside

Hospice nursing lets you care for the whole person: body, mind, and spirit. As an RN Case Manager at Infinity Hospice, you’ll be a trusted guide for patients and families during one of the most important times in their lives. You’ll bring clinical expertise, compassion, and steady leadership right into their homes.  If you want to do meaningful work, build lasting connections, and practice nursing as part of a team that truly works together, we’d love to meet you.

What You’ll Do

As an RN Case Manager, you’ll admit patients and provide nursing care to patients of all ages where they live. You’ll own the coordination of each patient’s care, working with the Interdisciplinary Group (IDG), the patient and family, and referring agencies. You will:

  • Assess patients thoroughly, using the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
  • Develop and document individualized plans of care based on your assessments.
  • Provide skilled nursing care using proper infection control practices (OSHA).
  • Partner with the IDG so every patient’s and family’s needs are met.
  • Coordinate with physicians, including attending physicians, hospice physicians, and others involved in each patient’s care.
  • Communicate clearly and compassionately with patients, families, team members, and other healthcare professionals.
  • Contribute to quality by taking part in the hospice’s QAPI program.
  • Share the hospice philosophy of comfort, dignity, and patient-centered care.

Requirements

Education and experience:

  • Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current State license as a Registered Nurse; Current driver’s license
  • Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills
