Job Overview:
Vitae Health Systems is seeking a highly experienced and dynamic Primary Care Operations Manager to manage the delivery of medical care across our skilled nursing facility partners. The ideal candidate, whether clinical or non-clinical, will have extensive long-term care experience and a proven track record in providing multi-site, multi-state healthcare operations. This role is crucial in maintaining the highest standards of patient care and operational efficiency throughout our network. The manager will focus on delivering exceptional clinical care, differentiating the company from competitors, driving growth (in terms of facilities, providers, visits, and geographic reach), and ensuring overall efficiency.
Key Responsibilities:
路聽聽聽聽聽聽聽 Oversee daily operations of primary care services across multiple skilled nursing facility partners, ensuring efficient, high-quality, and patient-centered care
路聽聽聽聽聽聽聽 Collaborate with providers, facility staff, and administrative teams to deliver cohesive and compliant service delivery
路聽聽聽聽聽聽聽 Conduct regular site visits and maintain strong relationships with facility administrators, clinical staff, and key stakeholders
路聽聽聽聽聽聽聽 Lead recruitment and onboarding efforts for your region, including interviewing, coordinating facility introductions, and ensuring readiness for new provider starts
路聽聽聽聽聽聽聽 Conduct structured performance reviews with providers and maintain consistent documentation and communication around expectations and improvement areas
路聽聽聽聽聽聽聽 Monitor clinical productivity and documentation compliance; partner with HR and clinical leadership to address performance or clinical concerns
路聽聽聽聽聽聽聽 Participate in monthly divisional reviews, presenting operational insights and clinician performance metrics to senior leadership
路聽聽聽聽聽聽聽 Drive strategic growth by identifying opportunities to expand services, optimize resources, and enhance operational efficiencies
路聽聽聽聽聽聽聽 Foster a culture of accountability, staff engagement, and continuous improvement across your region
路聽聽聽聽聽聽聽 Ensure timely and effective communication and follow-up with providers and facilities, maintaining a standard of response within one business day
Preferred Qualifications:
-聽聽聽聽聽聽聽 Multi-state experience highly preferred.
-聽聽聽聽聽聽聽 Master鈥檚 degree in Healthcare Administration, Business Administration, or a related field.
-聽聽聽聽聽聽聽 Clinical background (e.g., RN, NP) with experience in primary care or geriatric care preferred but not required.
-聽聽聽聽聽聽聽 Demonstrated ability to lead and inspire large, diverse teams.
-聽聽聽聽聽聽聽 Bachelor鈥檚 degree in Healthcare Administration, Nursing, Business Administration or a related field.聽
