Description

Ā 

Under the supervision of the Statewide Harm Reduction Manager performs duties relating to the HRC specific community case management and referral services as needed.Ā 

Tasks/Duties/ResponsibilitiesĀ 

70% Client ServicesĀ 

  • Provide HRC Case Management in designated county.Ā 
  • Conduct Community outreach, provider education and client recruitment of individuals in need of Harm Reduction Services. Ā 
  • Offer case management services to address obstacles while stabilizing at the HRC.Ā 
  • Conduct assessments to determine clients’ needs of supportive services.Ā 
  • Refer clients to HRC staff and/or community agency staff to assist them with psychosocial supports (e.g., housing, transportation, employment).Ā 
  • Follow-up with all referrals and clients to ensure all needs are being met.Ā 

15% Reporting/Monitoring Ā 

  • Establish, maintain and update client case records using approved agency forms and procedures. Ensure confidentiality of all records.Ā 
  • Prepare monthly reports in accordance with agency policies and procedures.Ā 
  • Maintain and update client information in required reporting system.Ā 

10% Hyacinth representation/advocacyĀ 

  • Attend and participate in staff meetings and supervisory conferencesĀ 
  • Maintain working relationships with other community providersĀ 
  • Observe established administrative procedures.Ā 
  • Provide written reports and maintain statistics as requiredĀ 
  • Represent the agency at community events, meeting and conferencesĀ 
  • Attend and participate in staff training opportunities.Ā 
  • Adhere to Hyacinth policies and procedures including client confidentiality provisionsĀ 

5% Training and certifications Ā 

  • Attend required opioid and harm reduction meetings Ā 

Other DutiesĀ 

  • Maintain working relationships with other community providersĀ 
  • Observe established administrative proceduresĀ 
  • Assist in preparation of written and electronic reports, maintaining records and statistics of program data as required.Ā 
  • Assist with group activities, outreach events and other program informal and formal social events. Ā 
  • An ideal candidate must have flexibility to work nights and some weekends as needed. Ā

  • Recognizes problems and complaints and escalates to team leaders and/or managers when necessary. Ā 

Requirements

Required Minimum Education: Bachelor’s degree, can be replaced with appropriate experience


Required Years of Related Experience: 1-3 years, experience with Harm Reduction/Social services/community services

Ā 

Required Knowledge, Skills and Abilities: Ā 

Ā· Active listening skills Ā 

Ā· Familiarity with trauma informed care approachĀ 

Ā· Knowledge of available services and community resources Ā 

Ā· Bi-lingual (English and Spanish) is a plus


People Management Experience: Ā 

Indicate whether supervisor/people management experience is required.Ā 

Supervisory experience is not required.Ā 


Preferred Minimum Education:Ā 

Bachelor's degree.

Candidate must have excellent oral presentation and writing skills.Ā

(Include any preferred computer skills, certifications, licenses, languages, etc.)Ā 

• Must be knowledgeable about working with people with active substance use disorders.Ā 

• Must be culturally competent and sensitive to the varying socio-cultural factors that contribute to substance use. Ā 

• Must be able to work non-traditional hours and days including late nights and weekends in order to reach high risk populations.Ā
