Per Diem Case Manager Opportunity - 

 

Pembroke Hospital is a 120-bed acute care, inpatient behavioral health facility located south of Boston, in Pembroke, MA.  Situated on 26 acres, we offer inpatient and partial hospitalization treatment to teens, adults and older adults, and our services are designed to assess, stabilize, and treat patients by addressing their primary symptoms and problems. The Case Manager is a vital part of Pembroke Hospital’s multidisciplinary treatment team. This is a per diem position. Program hours are Monday - Friday, 8:00am - 4:30pm. Pembroke Hospital is looking for a Case Manager who is available to work between 1-3 shifts (8am - 4:30pm), Monday - Friday.

 

Responsibilities

  • Conduct biopsychosocial assessments
  • Facilitate groups (strong emphasis on DBT/CBT skills)
  • Conduct family therapy sessions
  • Document clients’ treatment
  • Complete diagnostic assessments
  • Provide crisis interventions
